Job Description
Job Overview: Medical Social Worker
We are seeking a dedicated and highly skilled Licensed Social Worker for our client. The roles involves providing Social Work services to patients and families, formulating treatment plans, and ensuring continuity of care within the community or appropriate facilities.
Location: White Plains, NY
Compensation: $73,000 - $109,000/ year
Schedule: 8:00 AM - 4:00 PM, Monday to Friday
Responsibilities as the Medical Social Worker:
- Conduct interviews and complete comprehensive assessments.
- Ensure quality documentation of all services provided.
- Offer supportive counseling, crisis intervention, and assistance in adjusting to medical treatments.
- Collaborate with interdisciplinary teams to create and implement social work treatment plans.
- Provide referrals, advocacy, and liaison services with community agencies.
- Coordinate and participate in case conferences as needed.
- Supervise tasks for caseworkers, assistants, and social work students when assigned.
- Lead educational and support groups.
- Develop safety plans for victims of crimes/abuse.
- Maintain statistics of case activities and participate in departmental cross-training.
Qualifications for the Medical Social Worker:
- Minimum of 2 years of experience in an acute care hospital setting or similar.
- Master of Social Work Degree from an accredited School of Social Work.
- Licensed Social Worker (LMSW/LCSW) in New York.
- NYS SCREEN Certification (or obtained within the first 3 months of employment).
- Strong cooperative and constructive relationship-building skills with all stakeholders.
