Job Overview – Medical Social Worker (Bilingual Russian):
Take on a meaningful role as a Medical Social Worker with our client in Brooklyn, NY. In this position, you will provide critical psychosocial support to members, addressing social determinants of health, assessing needs, and removing barriers to care. As part of a collaborative team, you will ensure members receive compassionate and comprehensive services in alignment with their care plans. 

Compensation: $80,000 - $85,000/year
Location: Brooklyn, NY
Schedule: Monday to Friday, 9:00 AM - 5:00 PM


Responsibilities as the Medical Social Worker:
  • Psychosocial Assessments: Conduct initial and follow-up evaluations to identify members’ needs, coping strategies, and barriers to care.
  • Care Coordination: Gather social information from members, families, and agency staff to inform care planning and provide support during conferences and meetings.
  • Member Advocacy: Advocate for members by addressing social determinants of health and coordinating services to enhance their well-being.
  • Collaboration: Work closely with interdisciplinary teams to implement member-centered care plans, ensuring alignment with organizational standards.
  • Documentation: Maintain accurate and timely records of assessments, interventions, and member progress.
Qualifications for the Medical Social Worker:
  • Language: Fluency in Russian and English required.
  • Education: Master’s degree in Social Work from an accredited institution.
  • Licensure: Current NYS License to practice Social Work.
  • Experience: Minimum of 1 year providing social work services in an acute care or healthcare facility, with home care experience.
  • Skills: Strong analytical and communication skills to engage effectively with stakeholders and present clear information.
  • Attributes: Compassionate, detail-oriented, and committed to addressing social determinants of health.
