Job Overview – Medical Case Worker:
Become a vital part of our client’s team as a Medical Case Worker in Brooklyn, NY. This onsite position involves providing individual, family, and group counseling services to support families in need. Using evidence-based practices, you will address key areas such as child safety, family relationships, and caregiver support, collaborating with local resources and stakeholders to foster positive outcomes.
Compensation: $28/hour
Location: Brooklyn, NY
Schedule: Onsite, 35 hours/week
Responsibilities as the Medical Case Worker:
Family Support: Provide counseling and short-term crisis intervention for assigned families, addressing child safety and family dynamics.
Case Management: Conduct screenings and assessments for mental health, substance use, and caregiver capacity, ensuring monthly child assessments are completed.
Solution-Based Casework: Implement SBC principles, including action plans, documentation, and family-level outcomes, integrating practices into every counseling session.
Collaboration: Partner with ACS, schools, healthcare providers, and other resources to advocate for family needs and ensure coordinated services.
Documentation: Maintain accurate case notes, FASP documentation, and monthly reports in adherence with agency policies and databases (PROMIS, Connections).
Crisis Intervention: Address case-related crises with urgency and professionalism, including State Central Registry incidents and family court hearings.
Qualifications for the Medical Case Worker:
Education: Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Counseling, or related fields required.
Experience: Prior experience in child welfare required, with SBC training or certification.
Skills: Strong organizational, interpersonal, and counseling abilities with excellent inquiry and problem-solving skills.
Technical Skills: Proficient in utilizing databases such as PROMIS and Connections; familiarity with case documentation systems preferred.
Attributes: Commitment to a strengths-based, trauma-informed approach and youth/family development perspective.
Language Skills: Bilingual in English/Spanish preferred.
