Job Overview – Medical Case Planner (BSW):
Become an integral part of our client’s team as a Medical Case Planner in Brooklyn, NY. In this hybrid role, you will work closely with children and families in foster care, creating and implementing individualized service plans to promote safety, stability, and long-term well-being. This role emphasizes collaboration with multidisciplinary teams and community resources to help children find nurturing and permanent homes as efficiently as possible.
Compensation: $26/hour
Location: Brooklyn, NY
Schedule: Monday to Friday, 9:00 AM – 5:00 PM (Hybrid)
Responsibilities as the Medical Case Planner (BSW):
Family Support and Planning: Create comprehensive service plans addressing safety, behavioral, educational, psychological, and permanency needs in collaboration with families.
Case Monitoring: Conduct regular casework contacts to monitor safety, well-being, and progress, documenting all findings in progress notes and case documentation.
Collaboration: Partner with internal and external service providers and stakeholders to ensure effective support and clear communication for families.
Advocacy and Representation: Actively participate in Family Court Hearings, Family Team Conferences, and other meetings to advance permanency goals for children and families.
Qualifications for the Medical Case Planner (BSW):
Education: Bachelor’s degree in Social Work (BSW), Sociology, Psychology, or a related field is required.
Experience: Previous experience in child welfare, foster care, or case management.
Language Skills: Bilingual in Spanish is highly preferred.
Technical Skills: Proficiency in Microsoft Office and familiarity with NYS Connections or similar systems.
Attributes: Strong verbal and written communication, time management, organizational skills, and the ability to work with diverse populations.
Travel Requirements: Must be able to travel within the NYC Metropolitan area (all five boroughs).
