Job Overview - Medical Case Manager:
Become a dedicated Medical Case Manager in New York, NY, providing counseling, case management, and advocacy services to young adults (ages 18-24) experiencing homelessness. You'll support their transition to permanent housing while helping them gain job training, employment, and life-skills competencies.
Compensation: $28/hour
Location: Bronx, NY
Schedule: Flexible Shifts (Fieldwork across New York City required)
Responsibilities as the Medical Case Manager:
Case Management: Oversee the diverse needs of homeless young adults, providing counseling and group work services.
Assessment: Conduct biopsychosocial assessments to understand participants' strengths, risks, and challenges.
Action Plans: Develop and implement individualized service plans based on participants' specific needs and goals.
Crisis Management: Assist participants in managing crises and provide support with referrals for entitlements, legal assistance, and healthcare services.
Advocacy: Advocate for participants in securing housing, education, and employment, and assist them with navigating city systems.
Qualifications for the Medical Case Manager:
Education: Bachelor’s Degree required.
Experience: 2+ years serving at-risk youth, with a focus on assessments, counseling, and ISP development.
Specialization:Â Experience with LGBTQ youth and understanding the needs of young adults facing housing instability.
Skills: Excellent crisis intervention, problem-solving, advocacy, and communication skills, with a commitment to trauma-informed care.
Flexibility: Willingness to work flexible hours to meet participants' needs.
