Job Overview – Medical Case Manager (Medical & Social Services):
Join our healthcare client as a Medical Case Manager (Medical & Social Services) in Brooklyn, NY, delivering comprehensive case management, client advocacy, and service coordination to support individuals navigating medical and social service systems. This temporary role is perfect for a compassionate professional skilled in patient outreach, care planning, and community resource navigation, ensuring improved well-being and long-term self-sufficiency.

Compensation: $25/hour
Location: Brooklyn, NY
Schedule: Full-time (35 hours per week)


Responsibilities as the Medical Case Manager:
  • Client Intake & Assessment: Conduct initial and periodic assessments, establish program eligibility, and develop individualized service plans.
  • Case Management & Support: Maintain a caseload of clients, providing ongoing support, referrals, and advocacy for healthcare, housing, and social services.
  • Progress Monitoring & Documentation: Track client progress, update case files, and document service plan outcomes through regular follow-ups.
  • Client Advocacy & Outreach: Assist clients in overcoming barriers, advocate for services, and connect them with community resources to promote long-term stability.
  • Coordination & Compliance: Facilitate case conferences, liaise with external agencies, and ensure timely completion of reports and documentation.
  • Community Engagement & Education: Organize peer support events, conduct outreach to recruit clients, and provide educational resources on available services.
Qualifications for the Medical Case Manager:
  • Education: Bachelor’s degree in Social Work, Human Services, Public Health, or a related field required.
  • Experience: Minimum of 2 years of case management, social services, or healthcare-related experience, preferably working with individuals affected by HIV/AIDS or other chronic conditions.
  • Technical Skills: Proficiency in electronic case management systems and database entry.
  • Skills & Attributes: Strong interpersonal, problem-solving, and organizational skills, with the ability to work independently and as part of a multidisciplinary team.
  • Industry Knowledge: Understanding of Medicaid, social service programs, and community-based resources.
  • Language Requirement: Bilingual in English and Spanish or Haitian-Creole preferred.
