Job Overview – Medical Case Manager (Fieldwork):
Take on a transformative role as a Medical Case Manager (Fieldwork) in Philadelphia, PA, delivering critical support to individuals with severe mental illness and experiencing homelessness. In this full-time, field-based position, you’ll provide direct services and engagement, helping participants achieve stability and independence within the community.

Compensation: $24/hour
Location: Philadelphia, PA
Schedule: Monday to Friday, 9:00 AM - 5:00 PM


Responsibilities as the Medical Case Manager (Fieldwork):
  • Community Outreach: Spend 85% of your time in the community, providing direct services and building trust with participants across Philadelphia County.
  • Case Management: Assess participant needs, create and implement personalized care plans, and coordinate essential services for community integration.
  • Participant Advocacy: Support individuals facing severe mental illness and homelessness, addressing their challenges and connecting them to resources for long-term stability.
  • Compliance and Documentation: Ensure adherence to agency policies and behavioral health regulations while maintaining detailed records and reports.
  • Collaboration: Build professional relationships with behavioral health systems, community organizations, and internal teams to enhance service delivery.
Qualifications for the Medical Case Manager (Fieldwork):
  • Education: Bachelor’s degree in sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling, or education required.
  • Experience: At least 2 years of Case Management experience, with a working knowledge of mental health and substance abuse issues.
  • Skills: Strong communication, organization, and collaboration skills; ability to work independently in a dynamic field-based role.
  • Licensure: Valid driver’s license required for frequent local travel.
  • Attributes: Compassionate, adaptable, and organized, with a strong commitment to serving vulnerable populations.
