Job Overview – Medical Case Manager (Bilingual Spanish):
Take on an impactful role as a Medical Case Manager (Bilingual Spanish) in Brooklyn, NY. You’ll provide essential support to clients, addressing their immediate medical and social needs while helping them achieve long-term stability. This full-time position involves connecting clients to vital healthcare services, monitoring their progress, and ensuring compliance with program standards through accurate documentation.

Compensation: $26/hour
Location: Brooklyn, NY
Schedule: Monday to Friday, 9:00 AM - 5:00 PM


Responsibilities as the Medical Case Manager (Bilingual Spanish):
  • Client Advocacy: Conduct intakes and assessments, develop service plans, and connect clients to medical, mental health, and other essential services.
  • Progress Monitoring: Maintain consistent contact with clients through home visits and office meetings to track progress and update service plans.
  • Healthcare Coordination: Assist clients with accessing healthcare benefits, accompany them to appointments, and advocate with external organizations.
  • Housing Oversight: Monitor living conditions during monthly home visits, report repair issues, and collaborate with the Housing Specialist.
  • Documentation: Keep accurate, audit-ready client records, update program databases, and contribute to monthly reports.
Qualifications for the Medical Case Manager (Bilingual Spanish):
  • Language: Bilingual in Spanish required.
  • Education: Bachelor’s degree required.
  • Experience: Minimum 1 year of experience working with individuals facing homelessness, mental illness, or substance abuse.
  • Skills: Excellent written and verbal communication, organizational skills, and proficiency in Microsoft Office Suite.
  • Attributes: Detail-oriented, organized, and dedicated to empowering clients to achieve stability.
