Role Overview:
A healthcare organization is seeking a Nurse Practitioner to provide primary medical care and care management/coordination to a panel of assigned members residing in a contracted Nursing Facility. The ideal candidate will possess a graduate degree from a Nurse Practitioner program and have a minimum of two years of full-time experience as a Nurse Practitioner, preferably in clinical geriatric care.
Compensation: $120,000.00 - $160,000.00
Location: Bronx, New York
Schedule: Monday - Friday, 9 am - 5 pm
Responsibilities:
• Providing primary medical care and care management/coordination to a panel of assigned members residing in the contracted Skilled Nursing Facility
• Collaborating with the member's PCP and the nursing home Medical Director
• Conducting regular and acute visits/assessments in accordance with the Medical Staff by-laws of the Skilled Nursing Facility (SNF) and current Federal and State Health Code
• Working with the nursing home Interdisciplinary Care Team (ICT) members
Qualifications:
• Graduate degree from a Nurse Practitioner program
• Minimum of two (2) years of full-time experience as a Nurse Practitioner, preferably in clinical geriatric care
• Ability to work independently, develop a differential diagnosis, and initiate treatment
• Board Certification from AANP or ANCC
• Current DEA Certificate and Basic Cardiac Life Support (BCLS) certification
