Role Overview:
As a Nurse Care Manager at our client's nationally recognized care organization, you will provide coordinated whole-person care to members through in-home assessments, care planning, advocacy, and service authorization. You will collaborate across teams to optimize member health, independence, and outcomes.
Compensation: $95,000.00 - $110,000.00 Sign-on bonus: $20,000.00 Location: Remote - 1 day in the field
Schedule:
Monday - Friday from 9 am - 5 pm

Responsibilities:
Conduct member assessments and develop care plans
Authorize and coordinate covered services across care settings
Partner with members, families, and practitioners
Deliver personalized, proactive medical and social support
Champion interdisciplinary team collaboration
Qualifications:
Active NY RN license and registration
Bachelor of Science in Nursing degree
3+ years RN experience
Geriatrics, managed care, and behavioral health expertise preferred
