Job Overview – Licensed Social Worker (ACT Team Lead):
Take on a pivotal role as a Licensed Social Worker (ACT Team Lead) with our client in New York, NY. In this outpatient psychiatric setting, you will lead and supervise case management for individuals aged 18-25 with Serious Mental Illness (SMI). You’ll provide guidance to a dedicated ACT team, ensure compliance with policies, and deliver comprehensive support to help clients achieve stability and improve their quality of life.

Compensation: $65,000 - $75,000/year
Location: New York, NY
Schedule: Monday to Friday, 9:00 AM - 5:00 PM


Responsibilities as the Licensed Social Worker (ACT Team Lead):
  • Team Leadership: Supervise and guide case managers to ensure effective service delivery.
  • Case Oversight: Monitor client cases and implement individualized care plans.
  • Fieldwork: Conduct outreach and field visits to engage clients within their communities.
  • Client Support: Provide direct interventions and participate actively as an ACT team member.
  • Training and Development: Mentor team members and social work interns, offering SIFI instruction.
  • Crisis Management: Lead crisis interventions to ensure client safety and stability.
Qualifications for the Licensed Social Worker (ACT Team Lead):
  • Licensure: Licensed Master Social Worker (LMSW) required.
  • Experience: At least 3 years of experience working with seriously and persistently mentally ill populations, including young adults, with ACT team experience required.
  • Skills: Proficient in Microsoft Office Suite (Word and Outlook); familiarity with the AWARDS database is a plus.
  • Certification: SIFI (Supervisor in Field Instruction) certification required.
  • Attributes: Strong organizational and multitasking skills, with excellent interpersonal communication abilities.
  • Preferred: Bilingual skills are highly desirable.
