TAG MedStaffing is hiring a Licensed Practical Nurse (LPN) in Fairfield, NJ for our client. In this on-site role, you’ll provide comprehensive health and wellness care, coordinate treatment plans, and ensure compliance with medical and regulatory standards. You’ll oversee hospital transitions, train staff, and collaborate with healthcare providers to deliver high-quality patient outcomes.
Responsibilities as the Licensed Practical Nurse (LPN):
Health Management: Develop and implement individualized care plans addressing medical, nutritional, mental health, and medication needs.
Care Coordination: Collaborate with healthcare providers to ensure continuity of care and effective treatment implementation.
Liaison Role: Serve as the key contact between physicians, hospitals, and behavioral health teams to support integrated care.
Hospitalization Oversight: Manage hospital admissions and discharges, ensuring smooth transitions and proper follow-up care.
Staff Training: Educate staff on health topics such as insulin administration, diabetes management, and blood pressure monitoring.
Qualifications for the Licensed Practical Nurse (LPN):
Licensure: Valid and current New Jersey State LPN license required.
Experience: Prior field nursing experience required, with exposure to community-based or residential healthcare settings.
Technical Skills: Proficient in Microsoft Word, Outlook, and electronic health record (EHR) systems.
Industry Knowledge: Skilled in interpreting medical documentation, prescriptions, and care plans with attention to clinical detail.
