Job Overview – Licensed Practical Nurse (Care Manager):
Compensation: $60,000 – $85,000/year
Location: Austin, TX
Schedule: Monday to Friday, 9:00 AM to 5:00 PM

In this role, you’ll support patients directly, develop personalized care plans, and collaborate with cross-functional teams using the Healthy platform to improve outcomes. Responsibilities as the Licensed Practical Nurse (Care Manager):
Patient Onboarding: Enroll patients in the Advanced Primary Care Model (APCM) using AI tools to build trust and engagement.
Care Plan Management: Collaborate with physicians and patients to design and maintain personalized care plans in the Healthy platform.
Care Coordination: Manage services such as medication support, transportation, and follow-ups to ensure seamless patient care.
Patient Engagement: Conduct proactive outreach to keep patients connected and supported throughout their care journey.
Documentation & Compliance: Record all interactions accurately in EMR and AI systems while maintaining HIPAA and regulatory compliance.
Qualifications for the Licensed Practical Nurse (Care Manager):
Licensure: Active Licensed Practical Nurse (LPN) license required.
Experience: Prior experience in care management, case coordination, or patient-facing healthcare roles required.
Technical Skills: Proficient in EMR systems and open to leveraging AI-based tools such as ChatGPT and Healthy for workflow efficiency.
Industry Knowledge: Familiarity with geriatric and Medicare populations preferred, with an understanding of value-based care models.
Skills & Attributes: Highly organized, communicative, and proactive professional with a passion for innovation and patient-centered care.
Language Skills: Bilingual in Spanish preferred.
