Role Overview:
We are seeking a licensed practical nurse (LPN) to provide in-home nursing care to clients across various group home locations. Candidates must have at least one year of nursing experience, preferably with disabilities/hospice/home care. They must hold a current PA driver's license and have a reliable vehicle to travel between sites (mileage reimbursement provided).
Compensation: $38.00 - $42.00 / HR Location: Montgomery County, Pennsylvania
Schedule:
Monday - Friday 12:00 pm - 8:30 pm

Responsibilities:
Provide in-home care to clients in various group home locations
Deliver medications, attend to illnesses, provide bedside care, wound care, take vital signs, conduct assessments
Complete nursing duties as assigned at the main location, including rotating medication cart duty for a day program
Coordinate physicians and clinics at the program center per Health/Home Support Services direction
Deliver direct nursing care for those with disabilities and maintain proper medical records meeting nursing standards and regulations
Call the central number from the group home landline when leaving the shift
Qualifications:
Experience in hospice, home care, case management, long-term, care or intellectual/developmental disabilities
Current Licensed Practical Nurse license
1 year of recent nursing experience preferred
Genuine interest in serving those with disabilities
Valid Pennsylvania driver's license
Reliable personal vehicle (mileage reimbursement provided)
Ability to drive between locations
Able to lift up to 50 lbs, transfer clients, push/pull medical carts and wheelchairs
Proficient with Outlook, Email, Word
