Job Overview - Licensed Mental Health Counselor (LMHC):
Join our compassionate team in Brooklyn, NY, as a Licensed Mental Health Counselor. In this vital role, you'll provide life-changing support to clients, conducting comprehensive mental health assessments and developing personalized service plans. Your expertise and empathetic approach will empower clients on their journey to recovery and personal growth.

Location: Brooklyn, NY
Compensation: $35/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday

Responsibilities as the Licensed Mental Health Counselor:
  • Client Assessment & Service Planning: Complete initial and annual psychosocial assessments, administer appropriate screenings, and develop service plan goals.
  • Client Support & Medication Management: Provide individual and group counseling, monitor compliance with psychotropic medications, and assist tenants in obtaining medications.
  • Collaboration & Documentation: Coordinate with interdisciplinary teams, maintain accurate client records, and document all services provided in accordance with regulatory requirements.
  • Crisis Intervention & Hospital Coordination: Conduct hospital visits, advocate for tenants' emotional health, participate in discharge care meetings, and provide immediate medical care interventions when needed.
  • Program Involvement & Community Engagement: Participate in incident reports, case conferences, high-risk meetings, and the referral process for NYC 15-15 program; engage in community outreach.
Qualifications for the Licensed Mental Health Counselor:
  • Certification: LMHC required.
  • Experience: 2 years of work experience with individuals with HIV/AIDS, chronic mental illness, substance abuse, and homelessness preferred.
  • Language Skills: Bilingual in English/Spanish preferred.
  • Skills: Excellent oral and written communication skills, proficiency with MS Office and Google product suites, ability to manage and perform in a team approach, and strong organizational and time management skills.
  • Knowledge: Understanding of systems such as AWARDS, CAIRS, CARES, and commitment to culturally sensitive and client-centered service delivery.
