Role Overview:
We are seeking an experienced Licensed Graduate Social Worker (LGSW) to provide counseling and care coordination services at our adult medical day program. You will assess participants, develop treatment plans, and help address psychosocial needs.
Compensation: W2: $30.00 - $35.00/HR | 1099: $36.00 – $39.00/HR
Location: Montgomery County, Maryland
Schedule: Part-time (15 Hours/Month)
Responsibilities:
• Conducting screenings/interviews to evaluate appropriateness for program
• Identifying emotional and social needs and providing individual/group counseling
• Connecting participants with community resources and services
• Assisting with acclimation to program and care plan involvement
• Maintaining timely progress notes and care plan updates
• Participating in interdisciplinary team meetings and conferences
Qualifications:
• LGSW licensure in Maryland
• 1+ year experience with adults/seniors in healthcare
• Strong verbal/written communication abilities
• Knowledge of gerontology and relevant resources
