Job Description

Role Overview:
Join our client as a Clinical Social Worker in Long Island City, where you will perform mental health assessments, provide individual and group counseling, and collaborate with a team to ensure high-quality patient care. The ideal candidate will have an unrestricted New York State Clinical Social Worker license and a minimum of two years of clinical experience.

Compensation: $80,000.00
Location: Long Island City, New York

Responsibilities: 
•    Performing mental health assessments to determine diagnoses and establish treatment plans
•    Providing individual and group counseling and psychotherapy, as well as crisis intervention and conflict resolution
•    Completing medical forms and documentation related to patient care, including facilitating transfers and referrals to outside programs or psychiatric clinics
•    Making and tracking referrals to other mental health agencies and providers, and collaborating with specialists for consultation and referral purposes
•    Working with the intake team to determine the best course of action for each patient
•    Maintaining confidential case files and following clinical chart review policies and procedures
•    Providing workshops for families on communication and other family-related issues
•    Coordinating emergency responses, including minimizing patient's danger to self and others, communicating with EMS/FDNY, and facilitating referrals to psychiatric or medical emergency departments
•    Directing the course of the patient's mental health care and treatment plan within the scope of expertise
•    Encouraging health promotion, disease prevention, and health maintenance, and providing patient advocacy
•    Collaborating with Registration, Scheduling, and Medical Records departments to ensure appropriate patient scheduling, flow, and documentation procedures
•    Observing, implementing, and promoting federal regulations as defined by HIPAA and The Floating Hospital's policies and procedures

Qualifications:
•    Unrestricted New York State Clinical Social Worker license
•    Minimum of two years' clinical experience preferred
•    Familiarity with word processing and other computer applications
•    Ability to speak Spanish and/or other languages preferred
