Job Overview – LPN Field Nurse (Group Homes):
Compensation: $43/hour
Location: Montgomery County, PA
Schedule: Monday to Friday, 12:00 PM – 8:30 PM


Our client is hiring a Licensed Practical Nurse (LPN) for a 2nd shift field role supporting individuals in group homes across Montgomery and Philadelphia Counties. This position involves daily travel, medication administration, nursing assessments, and coordination with clinics and physicians. Ideal candidates have experience in case management, home health, or IDD care and are comfortable working independently in a community setting.

Responsibilities as the LPN Field Nurse:
  • Daily Field Visits: Travel to assigned group homes to provide medication administration, health assessments, and hands-on nursing care.
  • Clinical Support: Perform wound care, take vital signs, assess symptoms, and respond to the needs of ill individuals under supervision.
  • Care Coordination: Collaborate with physicians and clinics at the day program center and coordinate follow-up care as directed.
  • Medical Documentation: Maintain accurate and compliant medical records for each individual in accordance with nursing best practices and state regulations.
  • Program Center Coverage: Rotate on the medication cart at the central day program as needed, and complete end-of-shift reporting using designated protocols.
Qualifications for the LPN Field Nurse:
  • Licensure: Active Pennsylvania Licensed Practical Nurse (LPN) license is required.
  • Experience: 2+ years of clinical experience in case management, long-term care, hospice, home health, or working with individuals with intellectual and developmental disabilities (IDD).
  • Licensure & Transportation: Valid Pennsylvania driver’s license and reliable vehicle required (mileage reimbursement provided).
  • Technical Proficiency: Proficient in Microsoft Outlook, Word, and email systems, with the ability to navigate basic computer and documentation tools in a field-based setting.
  • Skills & Attributes: Compassionate and self-motivated, with the ability to work independently in the field and a strong commitment to supporting individuals with physical and developmental disabilities.
