Job Overview - Licensed Practical Nurse (LPN):
Looking for a compassionate and skilled Licensed Practical Nurse (LPN) to oversee the medical care of youth in residential and foster care settings at our Bronx location. You'll play a vital role in promoting and restoring the health of adolescents through comprehensive and compassionate nursing care.
Location: Bronx, NY
Compensation: $38-$40/hour
Schedule: 9:00 AM - 5:00 PM, Monday to Friday
Responsibilities as the Licensed Practical Nurse (LPN):
Conduct home visits and assess health needs.
Establish and maintain a medical home for adolescents.
Liaise with healthcare providers and coordinate care.
Provide emotional and psychological support to clients and families.
Promote client independence through education and goal setting.
Document and audit medical records for compliance.
Administer medications and monitor responses.
Implement and practice the Sanctuary Model tools for a trauma-sensitive environment.
Qualifications for the Licensed Practical Nurse (LPN):
Graduate of an accredited practical nursing program.
Strong nursing principles and practices.
Excellent communication skills.
Current CPR certification.
Ability to accompany youth to appointments.
Commitment to a trauma-informed care approach.
