Job Overview – Intensive Case Manager:
Make a meaningful impact as an Intensive Case Manager in Philadelphia, PA, working within the Behavioral Health Services Division to support clients in achieving independence and stability. This temporary role involves field-based service delivery, personalized care coordination, and advocacy to help clients thrive within their communities.
Compensation: $24/hour
Location: Philadelphia, PA
Schedule: Monday to Friday, 9:00 AM - 5:00 PM or 8:00 AM - 4:00 PM
Responsibilities as the Intensive Case Manager:
Service Coordination: Develop and refine individualized rehabilitation plans focused on social, vocational, educational, and self-care goals.
Fieldwork Support: Deliver 85% of services in the field across Philadelphia County, providing direct client assistance in community settings.
Crisis Intervention: Respond to urgent situations, coordinating necessary resources and individuals for immediate support.
Client Advocacy: Identify service gaps, recommend alternative strategies, and assist clients in navigating available resources.
Documentation & Compliance: Maintain accurate, timely records of client progress and services, ensuring adherence to regulatory and funding requirements.
Qualifications for the Intensive Case Manager:
Education: Bachelor’s degree in Human Services, Social Science, Healthcare, or a related field.
Experience: 1-2 years of Case Management experience working with individuals with mental illness required.
Skills: Excellent communication and organizational abilities, with a proven ability to work both independently and collaboratively within a focused team.
Technical Proficiency: Proficiency in Microsoft Outlook, Word, and Internet usage (electronic health record systems preferred).
Service Delivery: Demonstrated ability to meet direct care expectations, including accurate and timely documentation.
