Role Overview:
We are seeking a Case Manager to provide coordinated care and services to intellectually and developmentally disabled individuals in group home settings. This role will conduct assessments, develop treatment plans, and connect clients with critical resources.
Compensation: Up to $24
Location: Philadelphia, PA and Montgomery County, PA
Schedule: Monday - Friday, 8:30 am - 4:30 pm
Responsibilities:
• Conducting assessments and developing goal-oriented care plans
• Implementing and monitoring behavior treatment strategies
• Coordinating care services and community resource referrals
• Teaching independent living and social skills
• Ensuring regulatory compliance in care facilities
• Maintaining timely and comprehensive documentation
Qualifications:
• Bachelor’s Degree in human services, social work or related field
• 2+ years working with intellectually/developmentally disabled
• Understanding of coordination and case management
• Strong communication and facilitation skills
• Valid driver’s license
