Job Description

Job Overview – Hospice Registered Nurse (RN):
Join our client’s hospice team as a Hospice Registered Nurse (RN) in Stamford, CT, providing compassionate, patient-centered end-of-life care. This full-time role involves 4-5 home visits per day, where you will assess patient needs, manage symptoms, and collaborate with interdisciplinary teams to deliver comprehensive and supportive hospice care.

Compensation: $95,000 – $105,000/year
Location: Stamford, CT
Schedule: Monday to Friday (Full-Time)


Responsibilities as the Hospice Registered Nurse (RN):
  • Patient-Centered Care: Deliver skilled nursing care, pain management, and symptom control to enhance patient comfort and quality of life.
  • Assessment & Care Planning: Conduct comprehensive evaluations and develop individualized care plans aligned with patient and family needs.
  • Education & Support: Guide patients and families through disease progression, comfort care, and emotional well-being.
  • Interdisciplinary Collaboration: Coordinate care with physicians, social workers, and healthcare professionals to ensure a seamless approach.
  • Regulatory Compliance: Maintain accurate electronic health records (EHR) and uphold hospice care standards.
  • Bereavement & Family Support: Offer guidance and emotional support to families before and after a patient’s passing.
Qualifications for the Hospice Registered Nurse (RN):
  • Licensure: Active Registered Nurse (RN) license in Connecticut required.
  • Experience: 1-2+ years of hospice nursing experience, with home health experience preferred.
  • Skills: Strong clinical assessment, critical thinking, and decision-making abilities.
  • Attributes: Compassionate, patient-focused, and able to work independently in a home-care setting.
  • Technical Proficiency: Experience with electronic health records (EHR) for documentation.
  • Requirements: Valid driver’s license and reliable transportation for home visits.
