Job Overview – Hospice Registered Nurse (RN):
Join our client’s hospice team as a Hospice Registered Nurse (RN) in Stamford, CT, providing compassionate, patient-centered end-of-life care. This full-time role involves 4-5 home visits per day, where you will assess patient needs, manage symptoms, and collaborate with interdisciplinary teams to deliver comprehensive and supportive hospice care.
Responsibilities as the Hospice Registered Nurse (RN):
Patient-Centered Care: Deliver skilled nursing care, pain management, and symptom control to enhance patient comfort and quality of life.
Assessment & Care Planning: Conduct comprehensive evaluations and develop individualized care plans aligned with patient and family needs.
Education & Support: Guide patients and families through disease progression, comfort care, and emotional well-being.
Interdisciplinary Collaboration: Coordinate care with physicians, social workers, and healthcare professionals to ensure a seamless approach.
Regulatory Compliance: Maintain accurate electronic health records (EHR) and uphold hospice care standards.
Bereavement & Family Support: Offer guidance and emotional support to families before and after a patient’s passing.
Qualifications for the Hospice Registered Nurse (RN):
Licensure: Active Registered Nurse (RN)Â license in Connecticut required.
Experience: 1-2+ years of hospice nursing experience, with home health experience preferred.
Skills: Strong clinical assessment, critical thinking, and decision-making abilities.
Attributes: Compassionate, patient-focused, and able to work independently in a home-care setting.
Technical Proficiency: Experience with electronic health records (EHR) for documentation.
Requirements: Valid driver’s license and reliable transportation for home visits.
