TAG MedStaffing is hiring an RN Case Manager (Pediatrics) in Westchester County, NY for our client, supporting medical care coordination and nursing oversight for children within a residential care environment. This role will manage healthcare coordination, medication administration support, appointment scheduling, and clinical communication while ensuring compliance with healthcare regulations and organizational standards. The ideal candidate is compassionate, organized, and experienced working with pediatric, residential, or community-based patient populations.
Responsibilities as the RN Case Manager:
Care Coordination: Coordinate medical appointments, treatment plans, and ongoing healthcare services for assigned residents.
Clinical Oversight: Monitor resident health status, review medical documentation, and ensure compliance with healthcare and regulatory requirements.
Medication Management: Maintain medication administration records, coordinate pharmacy communication, and assist with medication distribution and education.
Direct Nursing Care: Provide nursing support and care for residents experiencing illness, injury, or other healthcare needs.
Family & Provider Communication: Serve as a liaison between clinical teams, healthcare providers, residents, and families regarding medical conditions and treatment updates.
Training & Documentation: Support staff training related to medications and healthcare practices while maintaining accurate and timely clinical documentation.
Qualifications for the RN Case Manager:
Education: Associate’s degree in Nursing is required, while a Bachelor’s degree in Nursing or related field is preferred.
Licensure: Active New York State Registered Nurse (RN) license is required.
Experience: 1–2 years of medical surgical, acute care, sub-acute care, or pediatric nursing experience is required.
Industry Knowledge: Experience within pediatric, residential care, behavioral health, or community-based healthcare settings is preferred.
Skills & Attributes: Strong communication skills, organizational abilities, attention to detail, professionalism, and the ability to coordinate care across multidisciplinary teams are required.
