Job Overview – Geriatric Social Worker (LMSW):
Compensation: $65,000 – $69,000/year
Location: Suffolk County, NY
Schedule: Monday to Friday, 9:00 AM to 5:00 PM
Our client is hiring a Geriatric Social Worker (LMSW) in Suffolk County, NY, to support elderly clients and caregivers impacted by Alzheimer’s and related conditions. This full-time role is ideal for Licensed Master Social Workers with experience in geriatric care, assessments, counseling, and caregiver support. You’ll coordinate services, lead educational programs, facilitate support groups, and work with families and community partners to improve quality of life for aging individuals.
Responsibilities as the Geriatric Social Worker (LMSW):
Client Intake & Assessment: Conduct comprehensive assessments to evaluate Alzheimer’s progression, safety risks, and client-specific needs; develop and implement intervention plans.
Family & Caregiver Support: Provide counseling, resource referrals, and education through in-person, phone, and online communication methods.
Community Outreach & Education: Lead caregiver trainings and educational programs for families, first responders, and healthcare professionals.
Support Group Facilitation: Organize and lead support groups for caregivers seeking peer support and guidance.
Representation & Collaboration: Attend external conferences and events, assist with special projects, and contribute to the broader mission of elderly care and support.
Qualifications for the Geriatric Social Worker (LMSW):
Education: Master’s degree in Social Work, Psychology, Counseling, Mental Health, or Gerontology required.
Licensure: Current LMSW license in New York State is required.
Experience: 2+ years of post-master’s experience working with geriatric populations, with a focus on Alzheimer’s or dementia care strongly preferred.
Technical Skills: Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
Skills & Attributes: Excellent interpersonal and communication skills, with strong organizational abilities and the capacity to work independently or collaboratively under pressure.
