Job Overview – Registered Nurse (Home Health):
Compensation: $95,000 – $105,000/year
Location: Stamford, CT
Schedule: Monday to Friday, 9:00 AM – 5:00 PM (Field-Based)


Deliver patient-centered care as a Registered Nurse (Home Health) with our client in Stamford, CT. In this role, you will provide clinical assessments, wound care, medication administration, and care coordination for patients across Fairfield County. You’ll execute physician orders, monitor patient progress, and collaborate with home health aides and care teams to ensure quality outcomes. This opportunity is ideal for RNs with strong home health experience, knowledge of OASIS, and a passion for providing personalized care in the community.

Responsibilities as the Registered Nurse (Home Health):
  • Patient Care: Conduct comprehensive clinical assessments and deliver nursing interventions, including wound care, medication administration, and skilled procedures.
  • Care Coordination: Collaborate with physicians, home health aides, and interdisciplinary providers to ensure integrated, patient-centered care.
  • Monitoring & Documentation: Track and document patient conditions, progress, and changes, ensuring accurate records.
  • Physician Orders: Execute physician-directed care plans and follow up on treatment directives.
  • Community-Based Nursing: Provide care in patients’ homes across Fairfield County, ensuring timely visits and quality standards.
Qualifications for the Registered Nurse (Home Health):
  • Education: Bachelor of Science in Nursing (BSN) preferred.
  • Licensure: Active Registered Nurse (RN) license in Connecticut required.
  • Certification: Basic Life Support (BLS) certification required.
  • Experience: 2+ years of nursing experience required, with prior home health experience strongly preferred.
  • Technical Skills: Proficiency in OASIS documentation and electronic medical record (EMR) systems.
  • Skills & Attributes: Strong communication, organizational, and critical thinking skills, with the ability to manage care independently.
  • Other Requirements: Reliable transportation required for patient visits across Fairfield County.
