Job Overview – Nurse Practitioner (Neurology & Chronic Care Management):
Compensation: $170,000 – $180,000/year
Location: San Francisco, CA
Schedule: Monday to Friday (Hybrid)


TAG MedStaffing is hiring a Nurse Practitioner (Neurology & Chronic Care Management) in San Francisco, CA for our client. This hybrid role supports patients with chronic neurological conditions such as Alzheimer’s and Parkinson’s disease through both in-person and telehealth care. You’ll collaborate with neurologists and clinical teams to deliver personalized, technology-driven care that improves outcomes and enhances quality of life.

Responsibilities as the Nurse Practitioner:
  • Patient Care: Deliver telehealth and in-person follow-up for patients with chronic neurological conditions, ensuring proactive, continuous management.
  • Care Planning: Create and update individualized care plans aligned with Medicare PCM/CCM guidelines.
  • Clinical Coordination: Collaborate with neurologists, primary care providers, and allied health teams to deliver integrated care.
  • Team Leadership: Supervise Medical Assistants, maintain clinical standards, and ensure adherence to care protocols.
  • Family Engagement: Support patients and caregivers with treatment adherence, medication management, and wellness education.
Qualifications for the Nurse Practitioner:
  • Licensure: Active, unrestricted Nurse Practitioner license in California required (additional state licensure strongly preferred).
  • Experience: 2+ years of experience in neurology, chronic disease management, or PCM/CCM programs.
  • Technical Skills: Proficient in Medicare billing codes, telehealth systems, and electronic health record (EHR) platforms.
  • Industry Knowledge: Strong understanding of neurodegenerative disorders, chronic care models, and interdisciplinary coordination.
  • Skills & Attributes: Compassionate, patient-focused clinician with strong communication and problem-solving skills, thriving in clinical and technology-driven healthcare settings.
