Compensation: $92,000/year
Location: New York, NY
Schedule: Monday to Friday, 9:00 AM to 5:00 PM


TAG MedStaffing is hiring a Registered Nurse (Critical Time Intervention) in New York, NY for our client. In this field-based role, you will provide behavioral health nursing, transitional care, and community-based support across supportive housing, shelter, and home settings. You will complete psychiatric assessments, coordinate care, support discharge planning, manage medications, and connect clients to essential health and social services to improve stability and long-term outcomes.

Responsibilities as the Registered Nurse (Critical Time Intervention):
  • Health Assessments & Transitions: Perform physical and psychosocial assessments, monitor changes, and support client transitions from inpatient to community care.
  • Care Planning & Medication Oversight: Develop individualized care plans, administer evidence-based interventions, and manage medication reconciliation and education.
  • Field-Based Support & Crisis Response: Provide in-home nursing services, emotional support, and crisis intervention to promote stability and reduce hospital readmissions.
  • Client Education & Resource Navigation: Counsel clients on disease prevention and wellness, and connect them with appropriate medical and social support services.
  • Collaboration & Documentation: Coordinate with multidisciplinary teams and external partners while maintaining accurate, timely clinical documentation.
Qualifications for the Registered Nurse (Critical Time Intervention):
  • Licensure: Active and unrestricted Registered Nurse license in New York State required.
  • Experience: Minimum of 1 year of post-licensure clinical nursing experience in the psychiatric or mental health field is required.
  • Technical Skills: Proficiency in documenting within EMR systems, conducting community assessments, and implementing individualized care plans.
  • Industry Knowledge: Strong understanding of psychiatric and medical medications, disease prevention, emergency response, and cultural competency in care delivery.
  • Skills & Attributes: Empathetic, adaptable, and team-oriented with excellent communication skills and the ability to build trust with vulnerable populations in field-based settings.
