Job Overview – Family Services Case Manager (Treatment & Rehabilitation):
Compensation: $25/hour
Location: New York, NY
Schedule: Monday to Friday, 9:00 AM to 5:30 PM


Take on a key role as a Family Services Case Manager in New York, NY, supporting families involved in the child welfare system. You’ll create service plans that promote family stability, behavioral health, and long-term safety while connecting clients to support services across New York City. This opportunity with our client is ideal for professionals with experience in case management, social services, or child protection and a strong understanding of underserved communities.

Responsibilities as the Family Services Case Manager:
  • Service Planning & Family Engagement: Work directly with families to build customized care plans addressing behavioral, educational, and permanency needs.
  • Ongoing Case Oversight: Conduct regular outreach and check-ins to monitor family progress, ensuring timely and accurate documentation.
  • Interdisciplinary Collaboration: Coordinate with internal staff, external providers, and city agencies to connect families with critical resources.
  • Court Participation & Team Meetings: Represent cases at Family Court hearings and multidisciplinary team conferences to advocate for positive outcomes.
Qualifications for the Family Services Case Manager:
  • Education: Bachelor’s degree in Social Work or a related human services field is required.
  • Experience: 1+ years working with at-risk populations, including those affected by homelessness, substance use, or immigration, with case management experience preferred.
  • Language Requirement: Bilingual in Spanish and English is highly preferred.
  • Technical Skills: Proficiency in Microsoft Office is required and familiarity with the NYS Connections system is highly preferred.
  • Skills & Attributes: Strong communication skills, attention to detail, cultural sensitivity, and a collaborative approach to family advocacy.
  • Travel: Must be willing and able to travel throughout New York City as needed.
