TAG MedStaffing is hiring a Director of Care Coordination in Brooklyn, NY for our client, leading high-volume home care staffing and compliance within a Licensed Home Care Services Agency (LHCSA). This role oversees coordination teams, ensures full case coverage, and maintains compliance with New York State regulations using systems such as HHA Exchange. Ideal candidates have 5+ years of home care leadership experience, including 2+ years managing coordination teams.
Responsibilities as the Director of Care Coordination (Home Care):
Team Leadership & Oversight: Lead and manage Care Coordinators and Supervisors to ensure performance, accountability, and documentation standards are consistently met.
Staffing & Coverage Management: Ensure full staffing of authorized cases, oversee live-in and split shifts, and respond quickly to callouts, emergencies, and high-priority cases.
Regulatory & Labor Compliance: Monitor overtime usage and scheduling practices to maintain compliance with NYS home care regulations and labor laws.
Cross-Functional Collaboration: Partner with HR, Payroll, Recruitment, Nursing, and Intake teams to resolve escalations and optimize coordination workflows.
Metrics & Quality Monitoring: Track key coordination KPIs, address caregiver and patient complaints, and implement performance improvement initiatives.
Qualifications for the Director of Care Coordination (Home Care):
Education: Bachelor’s degree in Healthcare Administration, Business, Nursing, or related field required.
Experience: 5+ years of leadership experience within home care operations, including at least 2+ years managing home care coordination teams in a high-volume environment.
Industry Knowledge: Strong understanding of New York State home care regulations, LHCSA compliance standards, labor laws, and scheduling best practices.
Technical Skills: Experience with HHA Exchange or similar home care scheduling platforms required.
Operational Expertise: Demonstrated success managing high-census or multi-region coordination teams with strong staffing optimization capabilities.
