Job Overview – Registered Nurse (Community Health Nurse):
Compensation: $50 – $53/hour
Location: Bronx, NY
Schedule: Tuesday to Saturday or Sunday to Thursday, 8:30 AM to 5:00 PM
Our client is hiring a Registered Nurse in Bronx, NY, to deliver field-based care for elderly and medically complex individuals in a community setting. Responsibilities include conducting UAS assessments, managing medications, developing care plans, and providing health education alongside an interdisciplinary team. Strong clinical judgment, effective communication, and a proactive, safety-focused approach are key to success in this role.
Responsibilities as the Registered Nurse (Community Health Nurse):
Comprehensive Assessments & Care Planning: Conduct holistic nursing assessments using UAS-NY tools and develop individualized care plans based on physical, mental, and environmental needs.
Skilled Nursing & Clinical Oversight: Provide nursing treatments, administer medications, and adjust care based on clinical evaluations and patient response.
Care Coordination & Collaboration: Work with Care Managers, IDT teams, and external providers to ensure smooth transitions and continuity of care across all settings.
Education, Advocacy & Risk Prevention: Educate participants and caregivers on health management, promote safe home environments, and advocate for early intervention when risks are identified.
Documentation & Regulatory Compliance: Maintain timely, accurate records and ensure all care activities meet state, federal, and organizational standards.
Qualifications for the Registered Nurse (Community Health Nurse):
Education: Degree from an NLN-accredited school of nursing required (BSN preferred).
Licensure: Current active license to practice as a Registered Nurse in New York State.
Experience: Minimum 1 year of clinical experience in certified home health, acute care, medical-surgical, or critical care settings required.
Technical Skills: Proficient in completing UAS-NY assessments, EMR documentation, and navigating regulatory guidelines.
Licensure & Travel: Must possess a valid NY driver’s license with appropriate insurance and access to a car for home visits.
Skills & Attributes: Strong clinical decision-making skills, excellent communication, and the ability to work independently in dynamic field environments.
