Job Description

Role Overview:
Join our client as a Registered Nurse Health Care Coordinator, responsible for medical review, oversight, training, and counseling for assigned locations. The ideal candidate will have a current Pennsylvania RN license, coordination of care/case management experience with developmental disabilities, and a minimum of 3 years of nursing experience.

Compensation: $55/HR
Location: Philadelphia, Pennsylvania 
Schedule: Monday - Friday, 8:30/9 am - 4:30/5 pm, with on-call rotation once every three weeks

Responsibilities: 
•    Monitoring and coordinating health care services
•    Assisting in developing competencies to allow individuals to be more involved in their healthcare needs
•    Educating and training staff on various medical diagnoses, procedures, and protocols
•    Collaborating with the interdisciplinary team to promote, maintain, and restore health, prevent complications, and provide optimal comfort for each individual
•    Staying informed about best practices related to health care service delivery for people with intellectual and developmental disabilities
•    Maintaining practices consistent with Horizon House Developmental Services policies, procedures, purpose, vision, and values
•    Developing and implementing health care services according to federal, state, and county regulations and requirements
•    Supporting individuals to become independent, make decisions, and live a meaningful, dignified life with valued social roles

Qualifications:
•    RN program (degrees/credits earned outside the US must be evaluated for equivalency) and High School Diploma or equivalent
•    Current Pennsylvania RN License in good standing
•    Coordination of care/case management experience with developmental disabilities required
•    Experience in medical-surgical care
•    Valid driver's license, reliable transportation, and valid automobile insurance or insured driver
•    Effective verbal and written communication skills
•    Minimum of 3 years of nursing experience
•    Availability by phone Monday through Friday, 8 am to 5 pm for full-time positions, and participation in on-call rotation
•    Strong computer skills with proficiency in Microsoft Office; experience with electronic health records a plus
•    Successful completion of the pre-employment process, including motor vehicle record and criminal background check
