Job Overview – Health Care Coordinator (RN):
Advance your healthcare career by joining our client as a Health Care Coordinator in Philadelphia, PA. This role involves providing medical review, oversight, training, and counseling for assigned locations, ensuring high-quality health care services for individuals with intellectual and developmental disabilities.

Compensation: $55/hour
Location: Philadelphia, PA
Schedule: Monday to Friday, Flexible Shifts (On-call rotation once every 3 weeks)


Responsibilities as the Health Care Coordinator:
  • Health Care Coordination: Monitor and coordinate health care services.
  • Competency Development: Assist in developing competencies for individuals to be more involved in their health care.
  • Staff Education: Train staff on medical diagnoses, procedures, and protocols relevant to the individuals supported.
  • Team Collaboration: Work with the interdisciplinary team to promote, maintain, and restore health and provide optimum care.
  • Best Practices: Stay updated on best practices for health care service delivery for people with intellectual and developmental disabilities.
  • Policy Compliance: Ensure practices align with developmental services policies, procedures, and values.
  • Service Implementation: Develop and implement health care services according to regulations and requirements.
Qualifications for the Health Care Coordinator:
  • Education: BSN degree required.
  • Licensing: Current PA RN License in good standing.
  • Experience: 3+ years in Nursing; experience with developmental disabilities and medical-surgical care.
  • Technical Skills: Proficient in Microsoft Office and experience with EHR a plus.
  • Transportation: Valid driver’s license and reliable transportation.
