Job Overview - Field Nurse (LPN):
Join our client in Montgomery County, PA, as a Field Nurse (LPN), providing direct care to individuals with developmental and physical disabilities in group homes. You'll deliver medications, perform assessments, and coordinate care with physicians and clinics.
Compensation: $38 - $42/hour Location: Montgomery County, PA
Schedule: Monday to Friday (2nd Shift)
Responsibilities as the Field Nurse (LPN):
Home Visits: Visit multiple group homes, delivering medications, performing bedside care, wound care, and vital signs assessments.
Coordination: Work with physicians and clinics to ensure comprehensive care for individuals with disabilities.
Care Management: Provide direct nursing care and maintain medical records in compliance with nursing practices and state regulations.
Field Work: Travel throughout Philadelphia to provide care and support to residents in group homes.
Qualifications for the Field Nurse (LPN):
Experience:Â 1 year of current Nursing experience (Hospice, Home Care, Case Management, Long Term Care, or IDD preferred).
Technical Skills: Proficient in computer applications such as Outlook and Word.
Attributes: Genuine interest in working with individuals with physical and developmental disabilities.
Requirements: Valid Pennsylvania driver’s license, reliable car (mileage reimbursement provided), and willingness to travel between homes.
