Job Overview – Family Case Planner (Treatment & Rehabilitation Services):
Compensation: $25/hour
Location: Bronx, NY
Schedule: Monday to Friday, 9:00 AM to 5:30 PM
Our client is hiring a Family Case Planner in Bronx, NY to support at-risk families involved in the child welfare system. You’ll develop individualized service plans focused on safety, permanency, and behavioral needs while coordinating resources across New York City. Ideal candidates have experience in case management, social services, or child protection and a strong understanding of vulnerable populations and support systems.
Responsibilities as the Family Case Planner:
Family Service Planning: Collaborate with families to create service plans addressing behavioral, educational, psychological, and permanency needs.
Case Monitoring & Documentation: Maintain regular contact with families, assess safety and progress, and ensure accurate, timely case documentation.
Cross-Agency Coordination: Partner with internal teams, external providers, and community resources to align care and support.
Court & Meeting Involvement: Participate in Family Court hearings, team conferences, and planning meetings to support child and family outcomes.
Qualifications for the Family Case Planner:
Education: Bachelor’s degree in Social Work or a related human services field is required.
Experience: 1+ years working with at-risk populations in homeless services, substance use recovery, or migrant support, with case management experience preferred.
Language Requirement: Bilingual in Spanish and English highly preferred.
Technical Skills: Basic proficiency in Microsoft Office is required, with familiarity in the NYS Connections system preferred.
Skills & Attributes: Detail-oriented with strong written and verbal communication skills, a flexible and team-oriented mindset, and a commitment to supporting underserved families across diverse communities.
Travel: Must be willing and able to travel throughout New York City as needed.
