Job Overview – Family Care Nurse (RN):
Step into a vital role as a Family Care Nurse (RN) with our client in Brooklyn, NY, where you will provide essential health care services and advocacy for individuals in Family Care homes. You will work collaboratively with care providers and healthcare professionals to ensure quality care and compliance with health standards.
Compensation: $53 - $56/hour
Location: Brooklyn, NY
Schedule: Monday to Friday, 9:00 AM - 5:00 PM
Responsibilities as the Family Care Nurse (RN):
Health Assessments: Conduct quarterly home visits and complete nursing assessments within one week of each visit.
Care Coordination: Serve as a liaison to coordinate referrals and ensure timely delivery of health services.
Documentation & Compliance: Complete Quarterly Nursing Assessments, Annual Care Plans, and maintain accurate, compliant health records.
Medical Advocacy: Manage consultation reports and advocate for individuals' health needs during appointments and calls.
Training & Education: Provide training to individuals and Family Care Providers to enhance their understanding and ability to access and deliver appropriate care.
Qualifications for the Family Care Nurse (RN):
Education: Bachelor’s Degree in Nursing (BSN) preferred.
Licensure: Current RN license in the state of New York.
Experience: At least 2 years of Nursing experience (OPWDD, home care, or skilled nursing preferred).
Skills: Excellent communication, cultural sensitivity, teamwork, and professional boundary management.
