Role Overview:
The Counselor is responsible for providing personal care services to patients needing assistance with activities of daily living and other support services. Main duties include personal care, grooming, meal preparation, accompanying to appointments, light housekeeping, errands, and more.
Compensation: $22.00 / HR Location: Manhattan, New York
Schedule: Sunday - Thursday from 9 am - 5 pm

Responsibilities
Assess client needs including substance abuse, health, mental health
Provide counseling, case management, and group education
Assist with self-care, interpersonal, housing, and vocational skills
Manage discharge planning
Document information and provide reports
Assist with activities of daily living and personal care
Perform housekeeping and assist with household maintenance
Administer medications properly
Communicate effectively and follow policies
Qualifications
Associate's degree; CPR & First Aid certified
1+ years experience as a home health aide
Experience with people with disabilities or mental illnesses
Knowledge of medical skills and tasks
Bilingual in Spanish preferred
Compassionate with good communication skills
Proficient with Google and Microsoft Office suites
Works well in teams
