Job Overview – Conference Facilitator (LMSW):
Step into an impactful role as a Conference Facilitator (LMSW) in Brooklyn, NY, supporting families enrolled in prevention programs. You’ll lead family team conferences (FTCs), ensuring each family’s voice is central to discussions on services, progress, and outcomes. This role emphasizes a strengths-based, trauma-informed approach, fostering collaboration between families and staff.

Compensation: $32 - $35/hour
Location: Brooklyn, NY
Schedule: 35 Hours/Week


Responsibilities as the Conference Facilitator (LMSW):
  • Family Team Conferences: Facilitate FTCs to assess services, review progress, and create actionable plans to support positive child welfare outcomes or service completion.
  • Model Implementation: Utilize the six-stage ACS-approved model for FTCs, promoting Solution-Based Casework, Functional Family Therapy, and Family Treatment/Rehabilitation methodologies.
  • Supervision: Oversee the Conference Scheduler, ensuring timely and effective coordination of all conferences.
  • Home Visits: Conduct home visits to facilitate conferences when necessary.
  • Preparation & Review: Evaluate family case records to assess needs, progress, and required participants ahead of conferences.
  • Documentation: Submit detailed and timely documentation of conference outcomes in PROMIS and Connections databases.
Qualifications for the Conference Facilitator (LMSW):
  • Education: Master’s degree in Social Work or a related field required.
  • Experience: Minimum of 2 years in a child welfare-related setting, facilitating groups or team meetings.
  • Skills: Proficiency in Microsoft Office (Outlook, Excel, and Word); familiarity with PROMIS and Connections databases is a plus.
  • Attributes: Excellent facilitation and communication skills, team-oriented, and committed to a strengths-based, youth and family development, and trauma-informed approach.
  • Language: Bilingual in English and Spanish preferred.
