Job Overview – Clinical Intake Coordinator:
Launch your career with our client in Philadelphia, PA, as a Clinical Intake Coordinator. In this role, you will implement individualized treatment plans for youth, facilitate psychoeducational group sessions, and ensure service coordination across various systems and agencies. You’ll play a pivotal role in managing referrals, tracking progress, and fostering the social, emotional, and educational development of consumers while collaborating closely with the Program Director and treatment team.
Compensation: $25/hour
Location: Philadelphia, PA
Schedule: Monday through Friday, 9:00 AM to 5:00 PM
Responsibilities as the Clinical Intake Coordinator:
Service Documentation: Complete and maintain accurate case management documentation via Credible Electronic Records, ensuring timely submission of progress notes and billing information.
Treatment Plan Implementation: Collaborate with the treatment team to execute service plans, provide direct interventions, and track consumer progress.
Interagency Coordination: Facilitate communication and coordination among service providers, schools, parents, and other stakeholders to ensure continuity of care.
Advocacy and Communication: Advocate for consumer needs, provide regular updates to the Program Director, and deliver clear communication to responsible parties.
Youth Engagement: Facilitate psychoeducational group sessions focusing on life skills and social growth while providing direct supervision and support.
Crisis Management: Address unusual incidents and routine activities through detailed documentation and follow-up with the treatment team.
Qualifications for the Clinical Intake Coordinator:
Education: Master’s degree in chemical dependency, psychology, social work, counseling, nursing (clinical specialty), or a related human services field.
Experience: At least 1 year (minimum of 1,820 hours) of clinical experience in a health or human services agency, preferably in a drug and alcohol setting.
Systems Knowledge: Familiarity with child-serving systems, including education, juvenile justice, child welfare, mental health, and vocational rehabilitation.
Skills: Strong communication, problem-solving, and analytical abilities with expertise in behavioral health systems, youth engagement, and regulatory policy comprehension.
Technical Proficiency: Excellent computer skills with the ability to navigate software programs and electronic health record systems.
