Clinical Coordinator (LMSW)

Job Description

Role Overview:
The Clinical Coordinator will oversee a team of Case Managers, providing clinical direction and leadership to ensure the highest quality care for clients. Reporting to the Assistant Director of Programs and Outreach, the Coordinator will promote the use of Harm Reduction, Housing First, and other best-practice interventions. The successful candidate will ensure client records are maintained in compliance with regulatory and agency standards.

Compensation: $64,000.00 - $68,000.00
Location: New York City
Schedule: Monday - Friday, 9 am – 5 pm

Responsibilities:
•    Supervise and provide regular structured meetings for Case Manager team
•    Promote effective case management strategies following Housing First, Harm Reduction, CPI, Motivational Interviewing and trauma-informed care models
•    Oversee development and implementation of individual service plans
•    Ensure timely and comprehensive documentation of client contact and progress
•    Interview and assess potential clients for program eligibility
•    Provide clinical guidance to troubleshoot client engagement and challenging behaviors
•    Foster a culture emphasizing housing placement as a measure of success
•    Assess and evaluate client functioning levels
•    Conduct and supervise case conferences
•    Coordinate care delivery with multiple service providers and outreach teams
•    Support staff in managing difficult situations and de-escalating conflicts
•    Perform other duties as assigned

Qualifications:
•    NYS Licensed Masters Social Worker (LMSW) required
•    Minimum 3 years experience with related populations
•    Previous supervisory and administrative experience preferred
•    Thorough understanding of Housing First, Motivational Interviewing, Harm Reduction
•    Experience with crisis intervention, risk assessment and incident management
•    Ability to work effectively with diverse internal and external stakeholders
•    Demonstrated success managing multiple priorities in fast-paced environment
•    Strong delegation and staff motivation skills
•    Excellent verbal and written communication abilities
•    Proficiency in Microsoft Office Suite (Word, Excel, Outlook, Teams)
•    Familiarity with Access databases and ability to learn new programs
•    Bilingual language skills preferred
