Role Overview:
As the Clinical Coordinator, you will play a pivotal role within our client's organization, reporting directly to the Assistant Program Director. Your primary responsibility is to uphold the highest standards of service provision.
Compensation: $64,000.00 – $68,000.00
Location: Brooklyn, New York
Responsibilities:
• Provide clinical leadership and direction
• Help troubleshoot and solve client engagement issues
• Manage staff and promote housing as a success within 90 days
• Oversee individual service plan development
• Ensure timely client progress documentation
• Assess the client's functioning
• Conduct case conferences
• Coordinate care with multiple service providers
• Collaborate with outreach teams
• Support clients in transitioning to permanent housing
• Provide guidance to front-line staff
• Assist with conflict de-escalation
• Perform assigned related duties
Qualifications:
• Master’s Degree in Social Work with an active LMSW license or MPH degree
• Minimum three years of experience with chronically homeless populations
• Housing-based case management experience
• Previous supervisory experience with strong clinical and assessment skills
• Proficiency in motivational interviewing techniques preferred
• Excellent collaborative skills for effective work with internal and external stakeholders
• Strong problem-solving abilities
• Demonstrated success in fast-paced environments
• Effective communication (verbal and written) with diverse populations and stakeholders
• Preferred: Bilingual in Spanish/English
