Job Overview – Clinical Assessment Manager (RN):
Take on a leadership role with our client as a Clinical Assessment Manager (RN) in New York, NY. In this position, you will oversee in-home and virtual assessments for participants in the PACE program, ensuring comprehensive evaluations and adherence to program standards. This role combines clinical expertise, case management, and a focus on quality improvement to enhance the care experience for elderly and frail populations.

Compensation: $110,000 - $115,000/year
Location: New York, NY
Schedule: Monday to Friday, 8:30 AM – 5:30 PM


Responsibilities as the Clinical Assessment Manager (RN):
  • Participant Assessments: Conduct in-home and virtual evaluations using UAS-NY to assess eligibility, care needs, and plan requirements.
  • Clinical Documentation: Update diagnoses, medication records, and care plans in the EMR, ensuring compliance and accuracy.
  • Care Planning: Create and implement service plans based on evaluations, collaborating with interdisciplinary teams.
  • Education and Advocacy: Educate participants and referral sources on PACE program benefits to encourage engagement.
  • Compliance and Reporting: Ensure adherence to CMS/PACE guidelines by completing documentation and following organizational policies.
  • Collaboration: Partner with Care Delivery teams to address participant needs and improve outcomes.
Qualifications for the Clinical Assessment Manager (RN):
  • Education: Bachelor’s degree in Nursing (BSN) required.
  • Licensure: Current active and unrestricted NYS Registered Nurse license required.
  • Experience: 1+ year with elderly or frail populations (PACE training available), 3-5 years in managed care programs, and 5+ years in care management or quality assurance.
  • Technical Skills: Proficiency in UAS-NY, Lenavi, Microsoft Office, and clinical management systems.
  • Language Skills: Bilingual candidates are strongly preferred.
  • Attributes: Strong communication, customer service, and clinical management skills, with the ability to work both independently and collaboratively in a fast-paced environment.
