Job Overview - Certified Diabetes Educator (RN):
Join our healthcare client as a Certified Diabetes Educator (RN), specializing in diabetic care and chronic disease management, in Collier County, FL. You'll provide education, preventative treatment, and comprehensive care while supporting interdisciplinary teams.
Compensation: $65,000 - $85,000/year
Location: Collier County, FL
Schedule: 8:00 AM to 5:00 PM, Monday to Friday
Responsibilities as the Certified Diabetes Educator (RN):
Diabetic Education: Teach patients about diabetes management, including medication, diet, and self-monitoring techniques.
Treatment Plans: Provide wound care and adjust treatment plans for diabetes patients.
Monitor & Assess: Regularly assess patients’ conditions, adjust medication, and provide ongoing care recommendations.
Collaboration: Work with physicians and the healthcare team to ensure seamless patient care.
Documentation: Maintain thorough records of patient care, assessments, and treatment plans.
Support: Engage with patients, families, and community partners to ensure understanding and adherence to care plans.
Qualifications for the Certified Diabetes Educator (RN):
Education: Bachelor's Degree in Nursing required (Master's preferred).
Experience: 2+ years in a Primary Healthcare setting.
Skills: Proficient in Microsoft Office (experience with Electronic Health Records preferred).
Language Skills: Bilingual in English and Spanish or Creole is a plus!
