Job Overview - Bilingual Case Planner:
We are seeking a Case Planner to work at our Bronx location. This role involves providing comprehensive case planning services for therapeutic children/youth and their families, with a focus on preventing children from entering the foster care system.
Location: Bronx, NY
Compensation: $48,000-$53,000/year
Schedule: 9:00 AM to 5:00 PM, Monday to Friday
Responsibilities as the Case Planner:
Conduct face-to-face contacts and home visits.
Develop and implement treatment plans.
Facilitate permanency and discharge planning.
Maintain case records and documentation.
Represent the agency in court.
Coordinate family and sibling visits.
Provide crisis intervention.
Collaborate on behavior plans and goals.
Participate in ACS meetings, team meetings, and conferences.
Assess family strengths and weaknesses for service planning.
Monitor and support foster parents.
Qualifications for the Case Planner:
Bilingual in English/Spanish required.
Minimum 2 years of experience working with youth and families.
Bachelor's degree in a related field required; Master's degree preferred.
Prior foster care experience preferred.
Ability to engage youth and families and establish working relationships.
Good written and verbal communication skills.
Must enjoy working with adolescents.
Commitment to strength-based, youth development, and trauma-informed perspectives.
Position requires travel for home and court visits.
