Job Overview - Case Planner:
Are you passionate about making a difference in the lives of youth and families? Our client is seeking an enthusiastic Case Planner to join their team in the Bronx and provide comprehensive casework services. In this rewarding role, you'll be at the forefront of child-centered, family-focused practices, working to promote permanency, safety, and well-being while helping reduce the length of stay in foster care. 

Location: Bronx, NY
Compensation: $26/hour
Schedule: 10:00 AM to 6:00 PM, Monday to Friday

Responsibilities as the Case Planner:
  • Case Planning: Maintain primary case planning responsibility for cases in collaboration with the social worker and permanency worker.
  • Family Contact: Establish immediate contact with families to provide information about the program.
  • Home Visits: Conduct home visits for observation, monitoring, and to facilitate reconciliation, reunification, and permanency. Assess homes for diagnostic purposes to ensure safety and complete home safety assessments.
  • Family Visits: Facilitate family visits, including visits with siblings in foster care.
  • Documentation: Write FASPs and Permanency Hearing Reports. Manage caseload of AWOL and suspended payment youth.
  • Community Relationships: Develop community relationships and communicate with all collaterals, including ACS.
  • Treatment Planning: Collaborate with the social worker and treatment team to develop the most appropriate permanency plan with an emphasis on lasting family connections.
  • Program Support: Perform other duties as assigned. Attend all program events and help prepare/clean-up for events.
Qualifications for the Case Planner:
  • Education: Bachelor’s degree required.
  • Experience: Experience working with families and adolescents in the child welfare system required.
  • Skills: Strong verbal and written communication, ability to establish supportive relationships, and knowledge of connections.
