Job Overview - Case Manager Supervisor (MSW):
Join our healthcare client in Staten Island, NY, as a Case Manager Supervisor overseeing a team working with older adults. This in-office role involves supervising case managers to develop and implement care plans for older adults, ensuring their needs are met.
Compensation: $70,000 - $74,000/year
Location: Staten Island, NY
Schedule: Monday to Friday (In-Office)
Responsibilities as the Case Manager Supervisor (MSW):
Supervision: Supervise up to 5 case managers (Master’s and Bachelor’s level) and assign cases as needed.
Authorization: Authorize EISEP homecare, housekeeping services, and home-delivered meals based on need and eligibility.
Client Caseload: Carry caseloads as assigned by the Program Director.
Compliance: Ensure staff adheres to DFTA standards and updates, and monitor data input into the STARS database.
Client Assessments: Conduct home visit assessments for high-risk clients when needed.
Qualifications for the Case Manager Supervisor (MSW):
Education: Master’s degree in Social Work (MSW) or a related Master’s level degree.
Experience: 2+ years in a Social Service Organization with older adults (supervisory experience in social work preferred).
Skills: Strong crisis management, communication, and team-building skills.
Preferred Experience: Experience with DFTA contracts and familiarity with the STARS database.
Languages: Fluency in a second language preferred.
