Job Overview - Case Manager:
We are seeking a Case Manager to work at our Bronx location. This role involves providing permanent supportive housing and case management services for adults living with HIV/AIDS and chronic medical conditions, as well as young adults aging out of foster care.
Location: Bronx, NY
Compensation: $24/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday
Responsibilities as the Case Manager:
Complete comprehensive assessments, service plans, and progress notes.
Continuously assess and monitor clients’ needs for referrals and services.
Provide psychoeducation on topics like Substance Abuse and Entitlements.
Facilitate individual and group workshops.
Collaborate with interdisciplinary teams to ensure comprehensive, client-centered services.
Qualifications for the Case Manager:
Bachelor's Degree in human services or related field with 2 years of case management experience, or
Associate’s Degree in human services or related field with 4 years of case management experience, or
High School Diploma with 6 years of case management experience.
Direct service experience with HIV/AIDS, substance abuse, mental illness, and homelessness.
Knowledge of HRA Congregate Housing Program guidelines.
Bilingual English/Spanish preferred.
Proficient in Google Workspace and Microsoft Office Suite.
Familiar with AWARDS, CAIRS, and CAPS systems.
