Job Overview - Case Manager:
We're seeking a dedicated Case Manager in Brooklyn, NY, to provide comprehensive case management services. The ideal candidate will conduct intakes, home visits, and follow-ups, ensuring clients receive the necessary support and resources.
Location: Brooklyn, NY
Compensation: $24/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday
Responsibilities of the Case Manager Include:
Intakes and Home Visits: Conduct initial intakes and regular home visits to assess client needs.
Follow-Up Calls: Perform follow-up calls and bi-monthly monitoring calls to ensure ongoing support.
Benefits Assistance: Provide assistance with benefits and entitlements to ensure clients receive all eligible services.
Referral and Linkage: Connect clients to other agencies and community resources for additional support.
Counseling: Offer counseling to support client well-being and improve caregiver/family relationships.
Qualifications of the Case Manager Include:
Education: Must have a bachelor's degree.
Language Skills: Bilingual candidates preferred in Spanish, French, Creole, Mandarin, or Cantonese.
