Job Overview – Case Manager (Behavioral Health)
Compensation: $28/hour
Location: Brooklyn, NY
Schedule: Monday to Friday (In-Office)

TAG MedStaffing is hiring a Case Manager (Behavioral Health) in Brooklyn, NY for our client, supporting housing stabilization, case management, and community support services within a residential social services environment. This role will assist program participants with accessing healthcare, behavioral health, housing, and community-based resources while providing ongoing support and advocacy services. The ideal candidate is compassionate, organized, and experienced working with individuals facing mental health, substance use, housing, or social service challenges.

Responsibilities as the Case Manager (Behavioral Health):
  • Case Management: Manage an assigned caseload while developing and monitoring individualized service plans for program participants.
  • Participant Support: Provide supportive counseling, crisis intervention, problem-solving assistance, and advocacy services.
  • Community Resource Coordination: Connect participants with medical, behavioral health, housing, vocational, and public assistance resources.
  • Home & Community Visits: Conduct home visits, hospital visits, and accompany participants to appointments as needed.
  • Documentation & Compliance: Maintain accurate documentation and ensure compliance with agency standards and program requirements.
  • Team & Community Collaboration: Partner with internal teams and community organizations to support participant engagement, recreation, educational, and community integration activities.

Qualifications for the Case Manager (Behavioral Health):
  • Education: Bachelor’s degree in Social Work or a related human services field is required.
  • Experience: Minimum of 2 years of case management, behavioral health, housing support, or related human services experience is required.
  • Industry Knowledge: Experience with case management, crisis intervention, community outreach, advocacy, and supportive services is preferred.
  • Technical Skills: Proficiency with Microsoft Office Suite, including Excel, is required.
  • Skills & Attributes: Strong communication skills, empathy, organizational abilities, crisis management skills, and the ability to work effectively with diverse populations are required.
  • Language Skills: Bilingual English/Spanish skills are preferred.
