Job Overview – Case Manager, Transitional Housing:
Our client seeks a compassionate Case Manager for their Transitional Housing Program in Brooklyn, NY. This role focuses on providing comprehensive support to individuals experiencing homelessness, including those with mental illness and substance use disorders, to facilitate their transition to permanent housing.

Compensation: $24/hour
Location: Brooklyn, NY

Responsibilities as the Case Manager:
  • Client Support: Manage a caseload of clients, conducting assessments, benefits assistance, and support to achieve housing and recovery goals.
  • Psychosocial Evaluations: Conduct evaluations and connect clients to psychiatric services as needed.
  • Benefits and Income: Assist clients in obtaining identification, public benefits, and income sources.
  • Weekly Meetings: Meet with clients weekly to assess needs, encourage harm reduction, and develop service plans.
  • Housing Applications: Prepare documentation for housing applications and counsel clients in obtaining permanent housing.
  • Crisis Intervention: Intervene in crisis situations and escort clients to appointments when needed.
  • Medication Assistance: Provide assistance with self-administration of medication, following organizational policies and procedures.
Qualifications for the Case Manager:
  • Education: Bachelor’s Degree or equivalent work experience.
  • Experience: 2 years of experience working with homeless adults and substance abuse issues preferred.
  • Skills: Experience with harm reduction and motivational interviewing techniques is strongly preferred.
  • Licensing: Valid NY or NJ driver’s license.
  • Technical Proficiency: Excellent computer skills, including proficiency in Microsoft Word and Excel.
  • Communication: Strong writing and communication skills; bilingual Spanish/English is preferred.
