Job Overview - Case Manager (Scatter Site):
Are you passionate about providing comprehensive support services to those in need? Our Bronx, NY location seeks a dedicated Case Manager to offer permanent supportive housing and case management services for adults living with HIV/AIDS and chronic medical conditions, as well as young adults aging out of foster care.
Location: Bronx, NY
Compensation: $24/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday
Responsibilities of the Case Manager Include:
Client Assessment and Planning: Conduct comprehensive assessments, develop service plans, and maintain detailed progress notes.
Referral and Monitoring: Continuously evaluate and monitor clients’ needs, providing necessary referrals and services.
Psychoeducation and Workshops: Deliver individual and group workshops, providing psychoeducation on substance abuse, entitlements, and more.
Interdisciplinary Collaboration: Work closely with interdisciplinary teams to ensure clients receive comprehensive, client-centered services.
Qualifications of the Case Manager Include:
Bachelor’s Degree in human services or related field with two (2) years of case management experience.
Associates Degree in human services or related field with four (4) years of case management experience.
High School Diploma with six (6) years of case management experience will be considered.
Knowledge of HRA Congregate Housing Program guidelines.
Direct service experience with individuals living with HIV/AIDS, substance abuse, serious mental illness, and homelessness.
Bilingual English/Spanish is a plus.
Proficiency in Google Workspace and Microsoft Office Suite.
Familiarity with AWARDS, CAIRS, and CAPS.
